*Improved patients are those who can pass a urinalysis demonstrating no use of addictive substances such as street illicit drugs like methamphetamine, cocaine, heroin, and fentanyl. *
Medication pickup and cost: All medications are prescription (Rx) from New Start Clinics and can be picked up from the pharmacy of the patient’s choice. The patient is responsible for covering the costs of their medication. New Start Clinics does not store or keep oral medications on-site.
Payment is due in full at the initial date of service. We do not accept payment plans for the following packages. All packages are non-refundable. Packages are for uninsured patients only. Patients with insurance must utilize their insurance benefits. Monthly packages are valid for 4 weeks from the time of purchase. By signing below, I acknowledge that I have read and understand the terms and conditions of this agreement.
